PET SITTING FORMS
In order to accomplish this goal, we will need you to supply us with crucial
information on your pet(s) and home. When you request service you will receive
our Pet and Home Care Profile forms. The forms may seem a bit daunting at first;
however, remember we will refer to these documents in your absence so please be
as detailed as possible. Rest assured that whenever you request service again all
the information will be in place and will not have to be done over. Promise!
Important: If there are any changes; new pets, illness, medications, lock
changes, alarm codes, people with permitted access to your home, etc., be sure to
let us know so we can update our files.
Important information and items we need include:
- Your pet(s) vaccination record. As the health and safety of all our animal
friends is a top priority with us, we're sure you will understand why we request
all pets to be current on their vaccinations before service starts. If you find
you're running short on time before your trip, we can take your pet(s) to your
vet for a reasonable fee if we're advised of your need early enough to schedule.
- The name, address and phone number of your preferred veterinarian and/or clinic.
- It is very important that we have your itinerary complete with your location and
all contact numbers where you can be reached while you are away.
- The name and number of at least one local emergency contact.
- Two copies of all keys needed to gain entry to your home.
OK. Now that you have gathered the phones numbers and other pertinent information and you
have copies of your keys and records in hand, it's time to move onto step two.
